Pre-Evaluation Meeting
Please complete the form above and send to Therapy Tails Ontario.  We will contact you with the date of the next pre-evaluation meeting

This meeting is mandatory and is  handler only
(NO DOG)

Please be prepared to complete a Volunteer Appication and a NRP Check

You  will be evaluated with your dog for fit and suitability to work with Therapy Tails Ontario. 

*Please note that both your photo and your dog's photo will be taken for your Picture Identification


Dog Requirements:
*at least one year of age
*all breeds and sizes accepted
*good temperment necessary
*up to date vaccinations

Handler Requirements:
*18 years of age or older
Vulnerable Persons Police Check necessary
*$40.00 one time set up fee

Additional info:
*you may have up to 3 dogs enrolled in the program

families are encouraged to participate during certain events
